Advance Care Planning Workshop:
Exploring Oregon’s Advance Directive and POLST

Have you heard about Advance Care Planning or ACP?

Advance Care Planning is a process that helps you to prepare in advance for your future medical needs such as when you are very hurt, sick, old or any other time that you are not able to fully communicate what you want or need.

Preparing for these situations beforehand through the ACP process provides you with some control over how you will be treated and cared for. It is also an invaluable gift to your loved ones who will not have to make these important decisions for you.

JOIN US as Dr. Onishi explains how the process works for both Oregon Advance Directives and Oregon POLST (Portable Orders for Life Sustaining Treatment). You will walk away with useful handouts & the tools to complete these forms! This is a free event.

Monday June 17th 10 am at the Redmond Senior Center
